- This document is the Lake County Comprehensive Annual
Financial Report (CAFR) which includes the financial statements of Lake
County, Florida prepared in conformity with generally accepted accounting
principles as applied to government units. The reporting entities include the

Sheriff, Tax Collector, Property Appraiser and Supervisor of Elections;

- This report is a summary of the cash and investment activity of all funds under the
control of the Board of County Commissioners. The report includes cash and investment balances by
amount, type and maturity, interest revenue earned and other information. The report is prepared
semiannually in accordance with the investment ordinance adopted by the Board and is prepared by the

County Civil

Tenant Eviction Package 0410.pdf (100k)
- This package is used by a landlord who wishes to
evict a tenant(s) for non-payment of rent for residential property only (this
package does not apply to commercial property nor mobile home parks).

Tenant Eviction Default Package - Count I - Possession.pdf (84k)
- This package is used after the Tenant Eviction case
has been filed with the Clerk, and the tenant has not moved or responded after
the five-day time period has elapsed. This package allows a landlord to obtain
a Final Judgment for Possession (of the subject rental property) directing the
Clerk to issue a Writ of Possession. The issuance of the Final Judgment is at
the discretion of the judge.

Tenant Eviction Default Package - Count II - Back Rent.pdf (13k)
- This package is used if Count II of the Complaint for
Tenant Eviction was requested, and the tenant has not responded within 20 days
after personal service of the summons. This package allows a landlord to
obtain a Default Final Judgment for Damages (Count II). The issuance of the
Default Final Judgment is at the discretion of the judge.

Court Payment Plans

Application for Criminal Indigent Status (25k)
- This form is used to determine if persons qualify for the services of the Public Defender in criminal cases. There is a $50.00 application fee for each form completed and reviewed.

Application For Determination Of Civil Indigent Status (24k)
- This form is used to determine if persons qualify to have the filing fee waived on a civil case or to enter into a payment contract for costs incurred on a civil case.

Payment Contract(17k)
- This form is used to set up a payment plan for fines/costs in felony, misdemeanor, criminal traffic, and civil traffic cases. There is a $25.00 administrative fee for each contract signed. All late fees and clearance fees must be paid before a payment agreement can be signed on a civil traffic citation.

Payment Contract For Restitution (17)
- This form is used to set up a payment plan for court-ordered restitution in felony, misdemeanor, and criminal traffic cases. There is a $3.50 administrative fee for each restitution payment made.

Civil Payment Contract (17)
- This form is used to set up a payment plan for any costs or service charges incurred on a civil case. There is a $25.00 administrative fee for each contract signed.

Blank Partial Payment Coupon (17k)
- These coupons can be used to mail in payments or to make payments in person for anyone enrolled in the Clerk's Office payment plan.

Request Form for Bank Account, Debit, Charge, or Credit Card Number Removal.pdf (72k)
- This document is used to request the removal of bank account, debit, charge, or credit card numbers
from court records. The request must be legibly written, signed, and delivered in person or by mail, facsimile, or electronic transmission to the Clerk/County
Recorder. The request must specify the bank account, debit, charge, and/or credit card number to be removed, as well as the
case number, document name, and page number that contains the numbers you want removed.
No fee is charged for this service.

Request Form for Records Search.pdf (215k)
- This document is used to request a search of Lake County public records. Complete this form and mail
or bring it into our office with payment of $2.00 per name per calendar year to be searched.
Payment must be received before the search will be completed. This search will not disclose records that
have been sealed or expunged. Allow seven to ten working days for return of results.

Official Records

Declaration of Domicile.pdf (5k)
- This document, when completed and recorded, is used to establish a domicile in the state of
Florida for persons who want to evidence the same by a recorded document. It is primarily used
by persons wanting to claim homestead exemption through the Property Appraisers Office. The
recording fee for this one page document is $10.00. If the person needs assistance from a clerk
in completing the information on the form, there is an additional $5.00 fee for this service
(per FS28.24(15)) as well as an additional fee of $10.00 if a clerk notarizes the form.

Official Records Order Form.pdf (25k)
- This document is to be used by non-commercial
entities only to request copies and/or certified copies of documents from the
Official Records Books. After printing and legibly completing this form, it must be mailed to Clerk
of the Circuit Court, Recording Department, P.O. Box 7800, Tavares, FL 32778, along with a check
or money order made payable to the Clerk of the Circuit Court. Copies are $1.00 per page
and $2.00 per document to certify (if certification is requested). There is also a postage and handling charge of
$1.00.

Recording Transmittal Form.xls (30k)
- Note: Must use Microsoft Excel or compatible program to open this form.
This form should be completed and included with documents submitted for recording
into the Official Records of Lake County. Although not required, it assists in providing
the Recording Division with the information necessary to record documents accurately and
in a timely manner.

Request for Confidentiality Form - Official Records.pdf (14k)
- This form is used by qualifying individuals to request
the removal/redaction of personal information on documents that are
maintained in the Lake County Official Records. The request must be legibly written, signed,
notarized and delivered to the Clerk of Courts Official Records Division.
The request must specify the identifying page number (Book and Page or Clerk
File Number) that contains the personal information being requested to
be removed. This statute does not allow for the removal of the
property legal description. No fee is charged for this service.

- This document is used to request the removal of certain sensitive document types from the Clerk's publicly
available website. Any affected person may request that images of the following types of
documents be removed from a Clerk's publicly available Internet website:

- Court files, records of paper relating to matters or cases governed by
- The Florida Rules of Family Law
- The Florida Rules of Juvenile Procedure
- The Florida Rules of Probate
The request must be in writing and may be delivered in person, by mail, electronically, or by facsimile.
The request must specify the identification page number of the document to be removed. No fee is charged for
this service.
Florida law requires that
images and copies of the above-listed documents remain on file and available
to the public upon request in the Office of the Clerk/County Recorder.

Emergency Child Pick-up Order.pdf (977k)
- This package should be used to request the court to enter an
order directing the sheriff or other law enforcement officer to take a minor
child(ren) from the person who currently has physical possession of the child(ren)
and deliver the child(ren) to your physical custody. This package
should only be used in an emergency by a person who has pre-existing legal
right to physical possession of a minor child. This means that you already
have a court order awarding you legal custody of the child(ren) or you

- This can be used in cases where you need to retrieve your property in an emergency. The Replevin action is used for

agreements with the Clerk's Office to have access to the Attorney Subscriber
Search site. This site provides subscribers with electronic access to indexes,
progress dockets, and images of non-confidential court records regarding cases
in which the subscriber is attorney of record. This service is provided at no charge.

- This form is used by persons who wish to exercise the
option of attending Driving School in lieu of having points assessed to the
driving record. Instructions for electing this option are found on the form.
The form should be submitted with a fee of $7.00 along with the original fine amount.

- This waiver is used by persons who find it impractical to provide proof of vehicle registration
or proof of security (personal injury protection insurance) as required by law. There is no fee
associated with the filing of this waiver.
