Court Payment Plan Forms

Application for Criminal Indigent Status

This form is used to determine if persons qualify for the services of the Public Defender in criminal cases. There is a $50.00 application fee for each form completed and reviewed.

Application for Determination of Civil Indigent Status

This form is used to determine if persons qualify to have the filing fee waived on a civil case or to enter into a payment contract for costs incurred on a civil case. This form may be downloaded from the Florida Supreme Court website or the Florida Clerk of Courts Operation website.

Payment Plan Application & Agreement

This application is used to set up a payment plan for fines/costs in felony, misdemeanor, criminal traffic, and civil traffic cases. There is a $25 administrative fee for each contract signed. For more information about court payment plans, click here.
